To request a copy of a previously submitted report not submitted through eFile, we require a written request signed by an officer of the company. The request must be on company letterhead and contain the name, signature, and title of the authorized requestor. The request may be submitted via secure message in eFile, mailed to the address on the form, or faxed to 301-278-9500.
Officers include the President, Vice President, Corporate Secretary, and Treasurer. Officers do not include the Corporate Controller, Financial Manager, or General Counsel, unless this person also holds the title of President, Vice President, Corporate Secretary, or Treasurer.
